ABSTRACT

This is a document presented to the Chilean Academy of Medicine by the Health Minister, the Economist Carlos Massad. First of all, he refers to the general context of the reform, that he considers a process and not a breakpoint. Afterwards, he enumerates the change tasks: to generate awareness of the change and to convert health in a State issue; to adequate programs to population's new health profile; to install new management instruments in the health system; to descentralize decisions and to promote the participation of beneficiaries; to establish incentives and to flexibilize the Health System and to introduce correctives to private health insurance. He believes that Chile must drift to a economical protection system for universal and solidary health events. The additional assurances that people wants to buy, must be delimited

ABSTRACT

O objetivo é analisar a atitude de dirigentes de organizacoes publicas e privadas de saude, no Estado de Santa Catarina. Caracteriza o tipo de acao de saude que ao Estado cabe adotar na implementacao de politicas de saude, detecta a atitude que predomina entre os dirigentes do setor saude, em relacao a mudanca proposta pelo SUDS e determina os padroes de mudanca predominantes no processo de implementacao do SUDS. Parte da elaboracao de um conjunto de afirmacoes que caracterizam e operacionalizam os tipos de acoes de saude que o Estado pode adotar (acoes de saude de carater publico, privado-complementar e privado) e caracterizam e operacionalizam os indicadores de mudanca do SUDS (descentralizacao, integracao, unificacao e participacao) e os padroes de mudanca (conflito, coalizao e negociacao). A verificacao empirica das questoes teoricas foi realizada atraves de pesquisa de carater exploratorio, envolvendo os dirigentes de primeiro e segundo escalao das organizacoes publicas de saude localizadas em Florianopolis, Santa Catarina, e uma amostra estratificada proporcional de dirigentes de organizacoes privadas de saude da cidade. Os dados foram obtidos atraves de entrevista estruturada e semi-estruturada
